Benjamin Ochan returned in goal after Charles Lukwago got ill, Isaac Muleme, Isaac Kirabira and Patrick Kaddu all starting.
The champions went into the game on the back of a loss and Mike Mutebi demanded reaction from his boys.
However, Mbarara City were organised in defence and thwarted all that KCCA threw at them.
The Ankole Lions are now 12th on the log with 15 points after 13 league games.
